Transcript/Records Request

In order to provide your school transcript to another institution/organization, the student (if over 18) or legal guardian (if under 18) must complete the release of information form. The form can be obtained by clicking here.
There are two ways to submit your completed form. You may hand deliver the form to the school and please make sure to bring proper identification with you. You can also mail or fax the form to the school, as long as your signature has been witnessed and notarized.
