SPED Transportation Mileage Reimbursement
This form is for parents providing their own transportation for special education purposes, approved by administration for mileage reimbursement. This form should be completed and submitted to the Superintendent's office by the last day of each month. Mileage will be reimbursed at the Federal Rate in effect at the time the transportation was provided.
